Finding the fragment

When you get into a forked path, what you need to do is keep moving northeast. Once you find the fragment of the map, which will be on the left side, then you will be good to go. Grand lift of Dectus Guide
When it comes to going to the Grand Lift of Dectus to unlock the Altus plateau in Elden Ring Map, you will first have to go to Liurnia on the northeast portion. When you have activated it, you will then have the access that you need to Altus Plateau. For the lifting process, you will be required to have two dectus medallions.
Locations of the medallions
The medallions will have to be found in the game, and you need to collect both of them, go to the grand lift, and then host your medallions so that you can activate the lift. It is not as hard as you think it is, so worries not, as you should be able to find it by following the steps below.
Left medallion
The left portion of the medallion can be found in Fort Haight, and this is on Limgrave, right on the southeast side. To enter, you will have to use the fort where some enemies will be waiting for you. If you are quick, you will be able to skip them. Then you will have to find your way to the roof, go up the ladder and go to the top of the tower. There, you will find the chest containing the left medallion.
Right medallion
The right portion of the medallion, on the other hand, can be found in Fort Faroth, a location within Caelid. The medallion should be easier to find by going to the left side of the castle, past the bats, and on top of the tower. There are not many enemies on this side, so it should be easy to figure out.
Once you get both medallions, you can go to the Grand Lift of Dectus, and then from there, you will be able to unlock the Altus Plateau in Elden Ring Map. You just need to follow all the steps above part by part, and you will just be fine.
